Right shoulder MRI

by butterfly26, May 14, 2008 08:23AM
Can some one help me out with this MRI report? Can you tell me in laymen terms what it is saying? I have had some (slight) discomfort in my right shoulder for the past few months so I thought I would have it check out.  With in 15 minutes of the doctor visit he gave me a steroid injection (right shoulder)said to do PT 3 X's a week and no work for the next 2 weeks. This all went down 3 weeks ago. He said the pain would be worse for the next 2 to 3 days but than it will fell like there was nothing wrong with it.  By that night I was in horrible pain.  I called the doctors office the next day because the pain was so bad and I couldn't even move a finger with out feeling like someone punched me (needless to say they did not return my call).  So Off to PT I went.  The physical therapist even called the doctors office because she had never seen anything like this, after a steroid injection.  They told her to tell me to take a couple of tylenol and it was just the burning from the steroid injection.  Well it did not get better and the slight discomfort is 110% worse and has been for the last 3 weeks. Last week the doctor ordered an MRI and said he would have the results sent to him STAT. After he got the results he said "do not move that arm put it in a sling for the next week" and to make an appointment to see him.   Here is what the MRI resutls said:
       INDICATION:

Right shoulder pain in a flight attendant, related to lifting heavy articles overhead.

TECHNIQUE:

Coronal T1, coronal and fat suppressed T2, sagittal T!, gradient echo and fat suppressed proton density weighted images of the right shoulder were performed with a shoulder coil in an open low field magnet.

FINDINGS:

There is edema in the muscle belly and myotendinous junction of the infraspinatus unit with a small area of what appears to be redundant tendon indication a mytendinous junction tear on the image 9 of series 7. The distal tendon it self is intact.  The remainder of the cuff is normal.  The subscapularis unity is normal.  The biceps tendon and anchor are intact.  The glenoid labrum is normal.  Glenohumeral alignment is normal.  There is no marrow edema or fracture.  The AC joint is normal.  There is no shoulder joint effusion or bursal fluid collection.  There is no mass.

IMPRESSION:

1." Partial tear at the myotendinous junction of the infraspinatus muscle with acute soft tissue edema but no disruption of the distal tendon identified".

2.  Otherwise normal MRI right shoulder

3.  Results discussed with Dr.

He said I have a tear in the muscle and that can not be surgically repaired and it was going to take time to heal.
So here is another question:  Why after a steroid injection did it get so bad?
I did go in for a second opoion and he (like the first doctor) asked if I had injuried that should and I said "not that I can remember"  He said with a small tear like that it could be possilbe that the needle nicked that muscle. Than he said what concerned him was all the fluid (edema) in that arm. And that it looked like someone punched me and I said nope and I do not recall hurting that shoulder at anytime in my life.  All he could say is " I have never seen anything like this and that is very weird" "hummm.....   That is really weird". We agreed to have another MRI done because he did not like the way the first one looked (it was an Open MRI not a closed one).

Help?????????  I am so frustrated and have been off work for 3 weeks now  and no one can tell my why, AFTER the steroid injection, would my shoulder would get 110% worse. They said I may be off work for another month......I can't afford that..............Not sure where to turn.
Can you help?
