I hope it's not another baker cyst

to whom it may concern,
my fiancee kelly has had surgury on her knees four times more then 6 years ago (she is now 25).. that said she has been working out at the gym and then she took a trip to new york but when she was there she started to notice that her knee was kind of tight, it didn't hurt but it was tight. so now she has been home for a week and we noticed that when she tenses up her leg a little mound of skin forms on the outside of her right knee.. I have no idea what it is but she has had the surgery in the past because she was a figure skater when she was young and then she had some baker cysts removed. yesturday it just formed but in how many days if it doesn't go away should we go to a doctor. Also should we use hot or cold on it to reduce the swelling... thanks very much you guys are the best
