First u/s, no heartbeat

by adgal, May 18, 2008 02:54PM
Hi Dr.

I have a history of miscarriage.  I am 39, and am now 6W2D pregnant again.  I am on prometrium (300mg/day orally), 600mg/day iron along with my prenatals.  I had an early u/s at 5W6D.  They saw a sac which was perfectly shaped (according to my Dr.), intrauterine, but no heartbeat.  It was measuring 5 to 5 1/2 weeks.  I have had no cramping or spotting, just one incident of a very quick, light bright red bleed, no tissue or clots.  My dr. is unconcerned, and is having me back in 4 weeks time, unless I have problems in the meantime.  I also have no real pregnancy symptoms other than mild on and off nausea, and extreme fatigue.  I can't seem to stop worrying.  I no no one can guarantee me a positive outcome, but am curious if you think all sound ok to you.  Does the  lack of hearbeat at 5W6D concern you, or the smaller sac measurement?  Thank you so much.  Oh, and I conceived naturally, and am certain of my LMP dates.  I also used an OPK, so I know I got my LH surge on day 11 of my cycle.  Last LMP was April 4th.

Hello,

The earliest a fetal heart beat can be detected by transvaginal ultrasound is ~5.5 wks gestation.  So, not seeing it when you were scanned is not concerning.

Also, progesterone is often prescribed in the situation you note, but has never been clearly demonstrated to be beneficial in preventing miscarriage.  That said, there is little down side beside the side effects affecting you as it will do no harm to the fetus.

Good luck and stay positive.

Doc opined that it is a intrauterine gestational sac at early pregnancy. Now at 7W also I am not feeling any symptoms instead of abdominal cramping. tommorrow is the  2nd U/s day for me. In some of the cases the H/B is not shown until 7 to 8 W period. So be possitive and happy dont worry. My prays are with u. bye
