Inability to ejaculate

During foreplay and sex, my partner has difficulty maintaining an erection and ejaculating. He is "turned on" but just as he feels he is going to ***, he feels as though there is a "blockage" and no ejaculate comes out. In order to bring him to the point of orgasm, he must use hand stimulation, as an erection is not maintained. Even so, he ejaculates once out of every 5 or six tries (maybe once ever few weeks). He has tried Cialis and Viagra without success. During oral sex, at the base of the penis (topside) I can feel a bubbly fluid (not sure if this is blood or blocked ejaculate?). This has been on ongoing problem since adolescense. He had a vericole repair in the Fall in hopes that this would solve the problem, however nothing has changed. He has Multiple Sclerosis, drinks alcohol, and smokes cigarettes, which yes I know, he should quit, but is unlikely to do so. Can you think of anything physically (a blokage of the "vas deferens" or something) that we might be able to look into in order to make it easier for him to reach orgasm and ejaculate? Also, we would like to try for a baby, however it is too difficult to get the timing down pat when he struggles with ejaculating... Please help.

I would guess he is still having some medical issue that is causing the problem. He might want to get a second opinion from a different doctor. There could be something his regular doctor is missiog.
