Heart falure and death

My grandmother is 60 years old and from what she tells me, she has been diagnosed with heart failure, high blood pressure,  and hypertension. Even with all this, she refuses to stop smoking, she smokes at least a pack to a pack and a half a day and she indulges in foods with salt/sodium enriched foods and she almost never takes her medicine (and I mean NEVER). All I want to know is, how long do you estimate she might live if these habits continue? Is it possible that she'll die in a year or two? Also, her symptoms include aching in her legs, feet, hands, back, and thighs; More symptoms are irritability and fatigue, she also seems depressed and will stay in bed for days without getting up and I bring her food. I just want to know, when is she going to die and how much can her health take, will she really live that much longer?

My Dad has had 4 heart attacks and also was told he was in end stage heart failure. They even put him on hospice care. That was a year and a half ago. He's no longer on hospice (there's a 6 month limit), he still smokes, eats terrible food and just sits around in his chair most of the day. I don't think there's any way to tell how long a person will live until they get down to the last couple of days. Some folks have bodies that can take a beating and keep going.
