cornual eptopic pregnancy - what next

A little on the history.  I felt pregnant in Feb (3 months ago) but 2 pregnacy tests were negative.  I then bled from late march for over a month, stopped for 2 days and started again.  The bleeding was initially put down to normal changes as I'm 40. I felt pregnant so did another test in early may which was positive so thought the pregnancy was recent and i must be miscarrying.  After checks I was diagnosed with the cornual eptop pregnacy and my HCG levels at that time was a little over 5000.  2 days later they had dropped so the consultant recommended a 'wait and see' approach as the pregnancy was to advanced for chemo and it was a planned pregnancy.  Since then the levels have steadily come down.  Last weekend i was admitted because of severe shooting pains. I was kept in overnight and observed.  My BP was quite low but stable.  I was discharged and had a scan/HCG test on Monday by a different consultant.  The HCG was a little over 1400 and scan showed mass still appeared - though definately no embryo.  This new consultant also muttered to the nurse there might be a fiboid there.  She signed me off work for another 2 weeks.  My original consultant later rang me back and suggested HCG checks 2 times per week instead of 3 and said all is going in right direction.  They confirmed I won't be having another scan and they'll just monitor my HCG levels from here on.    Since the weekend i've had mild/moderate period pain but no sharp pains. Sometimes i feel well/other times very unwell - ie wanting to defecate alot etc.  I've also regulary feel incredibly light headed / dizzy on and off.  Went to GP yesterday as didn't want to go to hospital again unless really necessary.  GP says i may now be miscarrying and given me some pain killers if i need them.  I feel fine again today but know that might not last.  But i understood the sac etc would just re-absorb into my body?  So if i'm miscarrying, should i see anything?  What do think happens now?  What questions should i be asking?  What about the sac and the mention of the fibroid - should they not want to check this again.  I'm confused as to what should happen next;  this feels like its been going on so long and i feel confused and anxious.
