2 1/2 year old all the sudden refusing to poop in diaper

I have a 2 1/2 yr old boy who has been refusing to poop in his diaper. He is no way ready to potty trained. I have seen the ped. dr. and she has recommended enemas, stool softeners, and laxatives. I am just wondering if there is other things that I need to be considering here? I am dealing with a behavioral issue? A power struggle? or a physiological problem? Or all three? Even when I have to resort to an enema my child shrieks when the urge to poop occurs and holds it until he can't hold it anymore. Help?!?!??!?

I have known some 2 1/2 to be potty trained...I don't think there is a problem...Just a stage he is going through...You should try a potty to see if that helps....If he still has a problem going...Miralax worked for my children....Did you doctor say anything else?...best of luck
