If his belly is abnormally large and his arms and legs are thin, and he's refusing to eat you probably should have him checked for a bowel obstruction. Has your doctor actually looked at him, or did you call in your concerns?
I really think you need to get his belly checked - if the doc still looks and says oh he's fine, try a second opinion.
