sudden behavior problems

I have a two year old grandson that I have kept since he was a few months old and recently I had a strokerecently which caused me to have to slow down alot. Because of this I see him almost every day but we are noticing a complete turn around with his behavior. He also has tubes in his ears and I wondered about "Middle ear Syndrome" I believe in spanking and when I spank I use a small thin paddle. His Mother WILL NOT SPANK_no matter how ugly he acts.
He was such a good child I don't understand what has happened and he was so happy before. He throws food at the table(even on his Mom) and he talks back telling us (NO) and he has a temper. Please advise this is so sad to see him this way.
