It does kind of sound like ectopic pregnancy. If there wasn't a good source of blood supply where the pregnancy implanted then it may be that not enough cell division was able to occur so that there's not much to see by ultrasound. This would cause infection and the antibiotics may have just fixed the symptoms but not the cause. If you get symptoms of infection again, it's definitely a good reason to go through with the laparoscopy.
Oh, and the relief from pcos symptoms after pregnancy is common. Sometimes pregnancy is even reported to kind of fix pcos. Like rebooting the hormonal system. I hope that this has happened for you and that this new development is just an acute complication that can be treated easily.
Anyway, please let me know what you find. You see, i have had severe pain in that same area for a few months now... but with other symptoms including bleeding on my period for 12 weeks now. It would be a great help if you could offer me a piece to my own puzzle.
