Potty trained 3yr old now wetting himself

My little boy is three and half yrs old, he potty trained himself on his 3rd birthday.

We NEVER put him under pressure to use the potty we just showed him how and one day he asked us not to put a nappy on when we were going out and bingo he was trained.

Last week (6 month on) he started peeing his pants and it was really strong smelling pee so I got a sample and brought it to our doctor, he couldnt find anything wrong but took another sample and sent it to the hospital , that smple came back clear again, in the mean time we put nappies on him as he was destroying himself at nursery and we didnt want him to be embarrassed.

Now he has started pooing in his nappy and the nursery says he'll have to leave as they only take trained children.

Could the doctor have missed something or is it him.

Im at my witts end as he has destroyed the house and car and now nursery.

Any suggestions?
