Bee sting like pain everywhere and pins and needles at back of head

My symptoms started 4 years ago after a shoulder surgery for arthritis and tendons sawn through. They have progressed over time in the following order. Had CAT of head 2 years ago which showed nothing. Lyme negative. My doctors diaganosis is Fibermialga(sp?) and problems from smoking. Had stopped smoking for a week but no symptom difference  Any help/advice would be appreciated.

Dizzyness - things going gray from bending position has progressed to happening anytime in standing position.

Breathing problems - Tests show COPD - treated with nebulizer and Advir. Breathing continues to worsen. I am a smoker.

Joints aching - started in knees, then ankles, now elbows. Take Sulindac which controls pain very well.

Back of head and arms falling asleep/pins and needles sensation when sleeping on back - head now gets pins and needles sensation while awake and standing.

Feet and legs pins and needles - from previous spinal surgery injury.

Bursts a few seconds of pain like a bee sting (at it's worst) usually in legs but has occured on torso and fingers. Happens hourly.

Pain like a cramp about to happen in calfs while sleeping.

Sudden pain in head/temples - lasts a couple of seconds.

Irregular/heavy heartbeat - extra beats - tests showed heart ok.

Insomnia and fatigue.

Pain under ribs on left side - occasional.
