delayed ejaculation

My boyfriend says he had some surgery when he was an  infant and doesnt know what the problem was but he can  make love for four or five hours straight and not have an orgasm.  He is only 42 years old, and says he has been this way his entire adult life, (since teen years).  What would be the cause of this?  He doesnt want to see a urologist and has no family to ask about the surgery,,, i do notice the opening of his penis is on the downward side of his penis, when i first saw his penis i wondered if he had  a botched circumcision as a baby,? but I did not mention it to him.  Any help you can be would be great, He does not  take any medications that I know of, he does drink alcohol, which normally would hurt the situation.  thank you again for all the help you can be.

Hi,

Surgical trauma, such as for hypospadias repair, can cause nerve injury.
As you have noticed that the urethral opening is toward the downward side of the penis, it is possible that he had a hypospadias repair.

Some amount of nerve damage or injury may have been sustained during the surgery which may have manifested as a late complication that did not receive medical attention because none was sought.

You can discuss this aspect with a urologist and a sex therapist to discuss treatment modalities deal with the retarded ejaculation.
