What is small vessel ischemia

I  have had headaches for years, in the last five years they have been unbearable i have taken medication had MRI scans that came up normal and change4d my diet nothing worked. I was diagnosed with trigemenalgia, then M.E. I felt like I was goig crazy I started to forget things, forget places and peoples names. I once was found on a bus not knowing where I was for a few minutes i became very stressed and had to call afriend on my mobile to come and collect me. Three months ago i had a TIA lost use of my right arm, weakness in right leg and right eye went very blurry like I was looking through a kalidescope. This lasted for about ten minutes, My eye was very small to look  at and my face disfigured and waited two months before an MRI scan. The reason for this in hospital they forgot to send me for one said "dont know how we missed you" it was also a bank holiday. the scan revealed I had small vessel ischemia. i have been given blood thinning medication, painkillers for my constant headaches and told there is nothing else to do? Prior to the stroke I had lots of pain in my head, I woke up one morning and my right arm was completely dead I had to lift it with my left arm this lasted for a few mins. Is there anything else that can be done to ensure the right diognosis and correct treatment, oh by the way they have lost my file!! and I have a hole in my heart. many thanks x
