Methotrexate can really have hematologic side effects, and your blood picture is consistent with such side effects. The drug primarily affects the white and red cells that can give rise to low levels of these. I suggest that you continue with your treatment, continue the blood work up monitoring and not to think so much about myelodysplastic diseases as we really have no concrete evidence of this being inherited (only less than 10% of cases).
