Physical symtoms certainly can cause a change in a persons' mood, but you said this has been going on for 5 years.
It appears he has several psychological issues that need to be dealt with.
I suggest he see a mental health provider for a thorough examination.
There are many options available, if, in fact his condition is related to his mental state.
We all have bad days and get angry sometimes, but consistent behavior of this nature is not normal and he needs some help.
