Need your opinion on whether surgery required or not for kidney removal

by Shivcharan, Jul 06, 2008 12:19PM
Hello...,I am Shivcharan an this is regarding my Dad who is 78 years old.A month ago he had a mild stroke and was
admitted to hospital for medication. He has partially recoverd from stroke (ie he can walk easily but only he has not
gained his left hand grip).During his stay in hospital he had the problem of urine retention and so catherterdral was
put for him.Later after sonography there was a stone detected in his right kidney. Now there was infection found in his
urine routine report and correspondingly doctor on doing the urine culture test could find the antibiotic for reducing the
infection.
However his stone size is 4.3* 3.0 cms in right kidney. There is no pain as of now nor earlier , so doctor had a doubt on
the functioning of the right kidney. Thus they asked the Digital Intra-Nenous urography test to be conducted.
The results indicated as below :
1. The first film taken at 3mins shows good nephogram on either sides.
The right kidney is enlarged as compared to left.
    Right kidney : 17.3*8.5 cms
    Left Kidney : 13.0*6.6 cms
2. Subsequent filsn taken show that there is a significant delay in excretion of the contrast on the right side with
optimum concentration seen in 24hrs. film which shows gross hydronephrosis.
The left pelvical system and the left ureter appear normal
The bladder is catheterised and appears normal.

Also to add the serum electrolytes test was done and the S.Creatinine count was found to be 1.1

Thus my question to you is :
1. Is there a serious need to do surgery for removal of Right Kidney or just removal of Stone would suffice and
    remove the infection.
2. We also would like to know  that any other test would be required to be done for confirming that no surgery is
    required for kidney removal.
3. Also looking at the patients age and history of stroke wouldnt it be better to first remove stone and then treat the
   kidney if infected.

Awaiting eagerly for your reply.

Regards
Shivcharan
