Epididymitis is an inflammation of the epididymis and can occur in response to an infecton or just local irritation. Infection can be bacterial or viral.
Infections can occur even with other bacteria and not just chlamydia or other organisms causing STDs. Some of these can even be the normal bacterial flora in the vagina.
As he already has an immunocompromised state of health, in view of diabetes, it would be better to have all infections treated (MRSA also) with effective antibacterial regimens rather aggressively.
Any existing infection can spread via the blood stream or by contaminated hands, clothes, etc.
It would be good to practice safe sex until the infection resolves as there is the likelihood that you may pick up an infection leading to an UTI at least.
Try to boost his immune system with sufficient vitamin A and C supplements, fluids over 2000 ml, and effective glycemic control.
Do keep us posted on your doubts.
