HPV?

Hello Doctor,

I had unprotected sex in December, 2006 (the only time I’ve had sex) several times with one partner over the course of a week or so. I knew the girl for a couple months and trusted her enough when she said she was clean and that she recently went to the college health center for a checkup.

Anyways, I remember experiencing muscle pain in the groin area about 1-2 weeks after for a few days before it went away. Perhaps a light rash of some sort in my inner thigh, but I can’t remember now. I also had a white pimple of sorts around the bladder area sometime after.. However, what most concerned me, and still concerns me, is the “wrinkly” skin part of my penis. I think it might be HPV.

I’ve been tested for Gonorrhea, Chlamydia, Herpes (about 2 weeks after incident) and HIV once I discovered she may have been more promiscuous than previously thought. I’ve also had the vinegar test done twice. I saw two different Staff Physicians at my health center, and 2 Urologists. Everything came back negative.

I know it may seem at this point that I’m simply neurotic, but the part that concerns me with the “wrinkly” skin part of my penis is that, since the sex, it’s been very dry/sensitive/frail (it’s uncomfortable to touch that particular skin of my penis); as I retract the skin it partially sticks to my inner penis head probably due to the dryness; but most importantly, I visually see slightly elevated almost cauliflower-like scales in the area, and there is—what seems to me—scarring right below it (about 1 inch in length, horizontally). The scarring concerns me from fear of it spreading, though it hasn't.

I was hoping it would go away, what seems to me cauliflower-like scales (the color of my skin) in the wrinkly-region of my penis, and the scarring, but it hasn’t since Dec. 2006. I also tried OTC anti-fungal creams, nothing helped. I was hoping it might be possible to take a picture of this region to share with you and for once and for all, to put my mind to rest?

Whoa!  All that STD testing after a single partner?  Not being a mental health professional, I can't comment on whether "neurotic" is the right word, but certainly this level of concern is not normal.  It doesn't sound like you are describing anything abnormal about the appearance of your penis.  Wrinkly skin is normal, and penile skin tends to get more wrinkly as men age, so it is natural it looks more that way than when you were younger.  The same comments apply to the dryness, "cauliflower"-like "scales", and everything else you describe.  This sounds like a classical example of someone looking too closely, not being familiar with the close-up appearance of penile skin, and concluding that something is wrong when in fact nothing is.  There is nothing in your descriptions that make me concerned you have HPV.  And if you happned to have caught HPV 18 months ago, it would have cleared up by now.  

In other words, I amn sure you have no STD, and I doubt there is anything abnormal about your penis.  But the way to know is to see a health care provider, frankly describe your concerns, and be examined.  (We don't examine posted photos on this forum.)  If something looks abnormal, the provider will know how to evaluate it further.  If s/he says it is normal, then you can put the issue aside and move on.
