Anal Fissure Surgery

I had anal sphincterotomy surgery about 3 weeks ago and I thought I would be feeling much better by now.  I've been following the diet and taking the medications as directed.  I no longer have excrusiating pain or bleeding while going to the bathroom but now have the following issues:

A few minutes after every bowel movement, I go into total spasm, my butt check is ultra sensitive near the surgery site and a part of this area is also rather hard.  I just finished a cycle of antibiotics (Flagyl) in case this was an infection. I still can't sit (still laying on my side) and walk like I just got off of a horse.  The throbbing is enough to make me feel like I'm going insane.

I've been applying presciption strength lidocaine ointment and most recently started applying Nitro Ointment into the anal canal.

I am looking for any advice:  Is this normal?  Should I be concerned?  I would greatly appreciate any feedback.
