I initially blamed the parents too, but after I've been told they have been to two different psychologists without improvements I thought it might be the psychologists incompetence.

it is common for children to fear pooping in the toilet, but that usually passes by the time they're 4. i did have one student (4) who was scared to poop in the toilet and always asked for a diaper. her parents had a great solution- they would put the diaper on her when she had to go, but made sure she did it while sitting on the toilet to get her used to the idea of sitting on the toilet. since she still had the security of the diaper she was ok with it. one day they cut a hole in the diaper, and sat her on the toilet as usual. the poop went through the diaper into the toilet. this helped her see that there was nothing to fear and from that day on, she used the toilet normally.
