recurring prostatitis

I’m 42 years old. I begin having problem with my prostate 2 years ago.
In the last 2 years I have had my prostate swell up or infected for at least 6-8 times. My Urologists would prescribe antibiotics. After taking the antibiotics it would be okay for a month to three months. After few months all the discomforts and pain would come back.
Discomforts start by first frequent urination. Sometimes I had to urine every 1.5 – 2 hrs. I would wake up 3-4 times in the middle of night to urine. Afterwards I would feel pressure from inside of my lower abdominal area, just like you would have gas and not being able to release gas. Eventually I would get shooting pain from my anus to my lower abdomen, right above my pelvic bone. At times when I am trying to pass gas I would be in so much pain right before passing it. It would be so hard to push until I get rid of a little pressure. Even having bowl movement was so hard.
Eventually the shooting pain would get so severe that I would end up in emergency room or urgent care. The whole process from discomfort, frequent urination and eventually escalated to unbearable shooting pain takes about 4-5 days.
The first time I was prescribed doxycyclean for 10 days. The second prescription was for 2 weeks, and third time for 4 weeks. I changed urologists and they have prescribed Cipro, and Bactrim, but I had allergic reaction to both. I have even taken Doxycyclean up to two months and after few months it also came back the same way. My Urologists suggested Flomax along with Doxycyclean, but my pharmacists, warned me Flomax might give me sexual dysfunction. Throughout this period I have seen over 5 different Urologists. I have done many urine and blood tests. I have had urinary traction Cystoscopy done. I have had also prostate biopsy done. All results came out okay and no sign of cancer or complication. After some internet research came across Supplement called Prostate care that cures prostatitis without having prescription side effects.
I tried taking it full dose 2 pills /day and I noticed it helped. When I would stop the supplement or reducing the dose to one pill/day it would come back after 4-5 weeks. As soon as I start taking the full dose again, it goes back to normal. I would have no pain and waking up middle of the night is to one time or sometimes not even once.
However, I noticed some side effects from this “Prostate Care”, herbal supplement, which is feeling of frustration, anger, short temper when I am on it for about a 3 -4 weeks on full dose of 2 pills a day. If I skip one or two dose straight for two or three days I would feel normal and go back on its full dose again before my prostate starts giving me problems. I have been trying this method for few months and it’s been working out okay. But now I am having sexual dysfunction which I never had before. It started with early ejaculation and recently I‘ve been having erectional dysfunction.
My Urologists is aware of all my symptoms and progress and he can not figure out either. He suggested Levitra to help my sexual dysfunction issue. He is amazed at the herbal supplement effects without the medication being so effective and he is the one who suggested skipping few dozes of the supplement to resolve my anger and frustration side effects. He said one of the ingredients of the supplement is small amount of estrogen hormones and that could be the cause of feeling irritated just like a female prior or after PMS.
I am frustrated with this situation that it keeps coming back and I do not have any solution without the side effects. My biggest frustration is that now I have sexual dysfunction and I am not sure it is caused by this herbal supplement or my prostatitis is causing it? Is this going to stay with me or it can be cured by some sort of medicine?

The effects of saw palmetto extract is still being researched on its efficacy in mitigating symptoms of prostatitis.

It has been found that the phytosterols present in saw palmetto may help reduce the symptoms of enlarged or infected prostate.

The basic suggestion at this point would be to stick to allopathic medication which can act specifically on your prostate, such as flomax, that has a known side effect profile which will be easier to manage.

Since sexual dysfunction was the primary reason you did not want to go on the flomax and you have not been able to avoid it with the herbal supplement, it would be better to try the Flomax along with a course of antibiotics.
The antibiotic can be decided upon after a prostate fluid examination and culture sensitivity. Prostate fluid can be obtained by a prostate massage test. Your urologist can explain the procedure to you.

After an effective course of antibiotics, you can settle down to treatment with flomax for a short while and see how your symptoms are doing.

Herbal preparations sometimes contain steroids which complicate the functions of other systems and will result in reduced immunity once you try to reduce or stop them. This could be the factor causing aggravation of symptoms.

If it does contain estrogen, it would definitely cause mood disturbances, in addition to other effects.

You will have to keep your fluid intake high, avoid unprotected sex, avoid prostate irritants, such as caffeine, alcohol, citrus fruits, etc., and try to incorporate natural sources of Beta-sitosterol, the active ingredient in saw palmetto extract. These natiral sources include avocados, wheatgerm, cashew fruit, pumpkin seeds, etc.

Sitting in a hot bath for 10-15 min twice daily will also help provide pain relief.
