4 year old split stream

I have a four year old daughter that has always had the same trouble since potty training at 2. She pees straight forward and not downwards into the potty. She does not have any trouble holding her urine... bedwetting etc..This upward stream causes a problem because she needs to take all of her bottoms off to potty-- awkward for her for school. She has been looked at and there is no visible adhesion or second hole according to the first doctors. She is scheduled for a VCUG and evaluation in a couple months. The anticipation of what exactly will happen at that appointment, as well as what may be the problem are the reason for my writing for advice. Can anyone tell me what may cause this and how it may be fixed.
