wanting another opinion about possible MS

I was wanting opinion about possible MS and if I should continue to persue more tests or wait until I have more symptoms. Also could this be something else?  Like Chronic Fatigue?  I have read the Chronic fatigue can have lesions also, not sure if true. also read could be caused by Mono and I have had Mono as a teenager.  Haven't felt completely right since.

I have been having the following symptoms. Slight daily headaches lasting for 1 month, they would last for a month or so and the I would have any for a couple of months,  last past 6-8 months I have been having headaches everyday, Usually only get severe headache about once a month or so,  I have tingling in both arms and legs from time to time and don't really notice them, feels like it itches. I have had trouble with fatigue for atleast the past 10 years or so, I have compensated with taking meds that give me energy such as diet meds.  Without meds, I yawn all day long, feel like I need to pull over when driving home after work because I'm so sleepy.  I would have to rest my arms while brushing hair, arms become weak. When I take medication, it doesn't seen to bother me.
Have had neck pain for about 10 yrs or so that is getting worse, gets real bad after work(i feel due to bending head to the right to hold a phone while talking).  have been throught physical therapist and spine MD,  I feel like he doesn't think I hurt and thinks that I'm only there for drugs.  The only narc I take is hydrocodone 5mg and only take a half when pain gets bad, recently haven't taken much, almost completely off by myself.  I have been given Wellbutrin for energy and it helped for a while.  recently started on Provigal, it helps.   I have occ Blurred vision but it doesn't last very long and don't know if it's my contacts, dry eyes or medications causes it. I have some floaters. I have been having a lot of memory problems.  I have to think a few minutes to remember how to spell words that I should know how to spell.  I work as a nurse and my MD will ask what is wrong with a patient that I triaged about 15-30 minutes later and I can't remember.  no bladder, balance problems.  has some numbess in legs if I cross my legs, causes foot drop,neuro says due to nerve behind knee, gets better after time. Right arm aches after a day at work but I think due to neck.


i have seen  2 neurologist(neither are MS specialist) and have had multipule test run. only says it's because of migraines, the other thinks it could be MS.  I like the second neuro better and he seems to listen and explains things.
I  also don't have faith in the PHD that did my Visual evoked potential.

see time line

8/2005 - (age 36) I was having dull headaches everyday for the past 1 month.  Usually very slight, occ bad, but everyday.  PCP wanted to do a MRI just in case.  She and I thought that it would be normal but she wanted to make sure there wasn't any tumors. Prior to the constant headaches, I only had a headache rarely.

8/4/05 - MRI showed "multipule white matter lesions. Questionable lesion in the right corpus callosum and several of these lesions are oriented perpendicular to the ventricles.  This is relatively young patient and the findings are more prominent than expected for age.  While these could represent small vessel ischemic type changes, other possibilities such ad Multipule Sclerosis are possible."

1/5/06 saw neurologist  - having tingling in upper and lower limbs, felt like an itch.  I have on both sides but usally on the left.

2/8/06 MRI reviewed by another MD, demostrates several predominately left, but also right periventicular and especially centrum semiovale what matter lesions. these are quite small.  measures 3-4 on the right, 6-7 on the left. only seen on the T2 Flair. Could be due to ischemic changes, demyelination lesions, gliotic changes such as vasospasms from migraines

? 2006, saw a second neurologist.  he ordered the following tests

?2006 -  had lumbar pucture-  normal.

5/11/06  (age 37)MRI  Brain - "nemerout abnormal white matter signal foci, mainly in the biparietal adn frontal regions within the deep white matter suggesting a demyelinating process. Clinical correlation and CSF assessment is suggested.

Spine MRI -  there is severe right lateral recess narrowing due to broad-based disc bulge and possibly some osteophyte formation.  Otherwise negative.

6/8/06 - Labs done, MS panel, hematology CFS, chemistry CSE and CSF VDRL -  all normal except for albumin was 8, (had gastric bypass 11/29/05)

2/5/07 MRI Thoracic spine - Spinal canal and lateral recesses are essentially patent. Vertebral body and disc height is well-preserved. Spinal cord signal and enhancement is unremarkable, with no focal lesion observed.

2/5/07 MRI Lumbar spine - no focal distal spinal cord lesion is seen.

2/5/07 MRI Cervical spine - Minimal degenerative disc disease at c5-c6, unchanged from prior study. otherwise unremarkable MRI.

2/5/07 MRI Brain - similar appearence of numerous white matter foci of abnormal signal, compared to 5/11/06.

7/1/08 X-ray cervical spine, spondylosis with no acute findings.

7/1/08 MRA Brain - The circle of Willis is incomplete, as the posterior communication arteries are absent. everything else normal.

7/1/08 MRI Brain -multipule foci of increased intensity within the deep and subcortical white matter of the centrum semiovale.  Demyelinating process such as multipule sclerosis to be considered. minimal paranasal sinus inflam. change.

7/1/08 MRI Cervical - c5-c6: broad-based disc protrusion which moderately narrows the ap demension the spinal canal as well as disc height reduction and disc desiccation.  left neuroforamin/normal, slight narrowing of the right neural foramen from vertebral osteophytes is seen.
)Spine MRI -  there is severe right lateral recess narrowing due to broad-based disc bulge and possibly some osteophyte formation.  Otherwise negative.

7/2008 Visual evoked potention - told it was on the "cuff".  stated right was normal and left was slighly slowed.  it was around 99 on one and 102 on the other to my best knowledge. He put a lot of wrong info in the report as far as my symptoms.

Were you tested for lyme disease?

Hi,

Please discuss with your neurologist the possibility of Sjogrens syndrome and migraine.
These  condition cause similar changes in the MRI and similar symptoms.
