thick endometrium no sac

okay ladies here is a update sister had an ultrasound done today. The just saw a thickened enodmetrium. The thing is some pregnancy test show positive others show negative. They didn't do a pregnancy test urine at the doctors office they just drew blood. according to her last menstrual period she will be 4 weeks 2 days is that the right thing to see at 4 weeks. Her breast aren't hurting yet. What do you guys think. help she is drving me crazy.... what would be other reasons for a thick endometrium??

I really don't know any other reason but I think it could just be too early to see anything. I would think seeing the thickened endometrium would be the first step to confirming she is pregnant, but honestly the doctor should have done a blood or urine test to be sure first.
