Herpes Transmission to Anus

Hello:

I have an unsual question and would like your opinion.  I have had HSV2 on my penis for 20 years - diagnosed through culture and repeated outbreaks over the years.  About 4 years ago, my wife performed oral sex on me - at the time I felt no presence of herpes.  She then used her tongue only (not lips) on my anus for about 5 minutes.  Several days later I had a pain deep in my rectum and at the opening of my anus but never had an outbreak in or near my anus then or since.  A herpes culture of my anus when I was in the most pain was negative.  Since that time, for 4 years, I have had nonstop rectal pain, including itching and sensitivity, and associated pelvic pain.  It is much worse when sitting and I am generally OK in the morning and progressively worse throughout the day.  Prior to the episode of oral sex, I had some pelvic and bladder pain and I had been diagnosed by a urologist with Interstitial Cystitis.  

I have since been diagnosed by a neurologist with pudendal nerve entrapment as the cause of my rectal, bladder and pelvic pain.  However, I am dubious about this given the timing between the tongue to anus contact and the anal pain about a week later.  

**So, my question really is:  Can a woman transmit herpes to the anus by performing oral sex on her HSV + partner and then licking his anus with just her tongue  -- thus causing nonstop postherpetic neuralgia -- or is it more likely that physical entrapment of the pudendal nerve is causing this 4 year nonstop anal pain?   Thank you in advance.

The likelihood that your pain is due to transfer of your own herpes from your penis to your anus via your wife's tongue is not plausible.  I can understand how the timing might raise the concern but I am sure it is merely coincidental.  There are multiple reasons I say this.  They include:

1.  Your body has developed a degree of immunity to further infection.  That helps to protect you from spreading infection from one place to another on your body.
2.  Herpes is not transferred from person to person through intermediates i.e. tongues, fingers, etc
3.  You describe the pain as non-stop.  The pain of herpes would be intermittent.

My advice- believe the neurologist.  Hope this helps.  EWH
