Is the skin condition localized on the head or glans of the penis or is the entire shaft involved as well?
An STD may be a differential here. I suggest that you have blood tests done just to rule this out. The scales may be microscopically evaluated so that fungal infections may also be ascertained if this is present. Balanitis is strong differential here. This skin condition is localized to the head or gland penis. This is usually caused by a fungal infection. However, dermatitic skin conditions may also be involved in balanitis. Have the scales evaluated first so that the right medications may be given. Applying any creams or lotions at this point may only cause further irritation without determining the cause of the skin condition.
The skin condition is only on the shaft, there is nothing on the glans.
Does that change the potential diagnosis?
If the skin condition is localized on the glans penis, then balanitis as a differential is likely here. However, if the shaft appears to be more affected, then balanitis may not be likely. However, a fungal infection, allergic or irritant dermatitis may still be considered. You have noted vigorous sexual activity prior to this. This could be contributory. This may not necessarily be an STD however, STD's still need to be ruled out here. The way you described the penile skin condition suggests dermatitis or a fungal infection. Microscopic evaluation of scales may help determine if a superficial fungal skin infection is involved.
