Scars and irritation on penis

Three weeks ago I was with my girlfriend (who lives abroad) and after having sex with her (perhaps six-seven times over 24 hours), as I was coming back and over the next few days I noticed some irritation on the shaft of my penis, particularly just underneath the glans. Irritation extended over stretches of 2-3 mm or less. This had largely subsided when I next saw my girlfriend, one week ago, although some light marks remained. This time however irritation and skinburn appeared during sex. After 24 hours sex became very painful. In fact, the skin was peeling around the areas where I had this irritation, producing a sensation of burn even without any rubbing. As of now, what remains is a few spots where the skin has a lighter colour than the rest of the penis shaft, plus some remains of peeling, and one little scar (2mm) just underneath the glans. I'm worried that this could be some sort of STD (and if so how and when that might have gotten there...) - but perhaps it is some other kind of dermatological problem? Any insights would be appreciated.

Is the skin condition localized on the head or glans of the penis or is the entire shaft involved as well?

An STD may be a differential here. I suggest that you have blood tests done just to rule this out. The scales may be microscopically evaluated so that fungal infections may also be ascertained if this is present. Balanitis is strong differential here. This skin condition is localized to the head or gland penis. This is usually caused by a fungal infection. However, dermatitic skin conditions may also be involved in balanitis. Have the scales evaluated first so that the right medications may be given. Applying any creams or lotions at this point may only cause further irritation without determining the cause of the skin condition.

Hello,

The skin condition is only on the shaft, there is nothing on the glans.

Does that change the potential diagnosis?

Hi

If the skin condition is localized on the glans penis, then balanitis as a differential is likely here. However, if the shaft appears to be more affected, then balanitis may not be likely. However, a fungal infection, allergic or irritant dermatitis may still be considered. You have noted vigorous sexual activity prior to this. This could be contributory. This may not necessarily be an STD however, STD's still need to be ruled out here. The way you described the penile skin condition suggests dermatitis or a fungal infection. Microscopic evaluation of scales may help determine if a superficial fungal skin infection is involved.
