"Paruresis (pronounced /pærjəˈriːsɪs/), also known as pee shyness, shy kidney, bashful bladder, stage fright, urophobia or shy bladder syndrome, is a type of phobia in which the sufferer is unable to urinate in the (real or imaginary) presence of others, such as in a public restroom. It can affect both males and females. The analogous condition that affects bowel movement is called parcopresis"

The testes can ascend toward the abdomen in times of stress, or in response to various stimuli, such as cold, strain, etc.
An enlarged prostate can cause difficulty in urination and can also cause pain in the groin.
You can try different medications that relieve the prostate-induced difficulty in urination. These include alpha blockers and 5alpha reductases.
Timed voiding, OTC agents like saw palmetto, avoidance of prostate irritants, such as caffeine, alcohol, citrus fruits, and spicy foods, can also help reduce the problem.
