Lathergic....unusual feelin in the tongue

My brother in law (age-38) has complaints that he noticed from last 7-8 days, something unusual feeling( dryness. etc) in the tongue with foul smelling, feeling like fever (but thermometer doesn't show any high temp.) with mild cough & yellowish sputum, his CBC report doesn't give any pathological abnormalities except ESR which is 58 in 1st hour.
He also did a Urine R/M/E exam (Normal findings), RBS 5.1 mmol/lit and X-Ray Chest P/A view, which told that "his rt. lung field shows mild pleural thickening", otherwise normal chet skiagram.
Many years ago ( near about 30 years) he suffered from Pulmonary TB and it was very late diagnosed and pleural effusion also develop[ed as its complication than he was brought to hospital and treated conservativelly. He is a smoker and usually takes 10-12 sticks/ day.
About 5 years ago he was very obese(body wt was 110kg), now a days he is 78 kg, he lost his wt. by heavy physical esercise and regular walking, he also restrict his daily meal to loose wt. his hight is 6 '.
Now, please told me why does he feels this type of symptoms? And what will i have to do for releaving him from his present health condition?
