Burning sensation on tip of my pennis

Dear Docorts
I am observing contiuous Burning sensation on the Tip of my Pennis, And a different type of pain while urinating, as if i am forcing to urinate. I have continuous urinating sensation but facing a problem in passing the urine.
Additional details
I am not involved in sexual activities with any one
I masturbate 2 - 3 times daily
I drink lots of Tea and Coke
I smoke 15-20 cigarrettes in a day
No Adequate sleep
Work on computer 18 hours
Please help me !!
