I would like to help, but really cannot offer much.  If your doctor can't find anything wrong to explain your symptoms, especially after such an extensive evaluation (including testicular ultrasound), there is nothing a distant online expert can do.  If we had a patient with concerns such as yours in my STD clinic, and thought he needed further evaluation, we would refer him to a urologist -- this isn't the sort of thing that STD experts manage themselves.

I will just say that your symptom do not suggest continuing chlamydia, NGU, or other STD, and your test results confirm that conclusion.  More than anything else, what you describe sounds most like anxiety-related symptoms, i.e. stress or other psychological factors magnifying normal body sensations.  Given your negative examinations and test results, you can be certain that you have nothing that ever will be harmful, either to you or a future sex partner.

In other words, perhaps the best thing is to just go on with life, knowing nothing serious is going on.  Most likely, your symptoms will just fade with time.  If that doesn't happen, continue to work with your current providers about next steps in evaluation.

Best wishes--  HHH, MD

"I know pain when I feel it" does not conflict with "all in my head".  Psychologically generated or exaggerated pain is real pain.  Since your own provider believes it, you should too -- and you should accept his advice about it.
