what treatment options?

by stillsmiling1015, Jul 24, 2008 09:07PM
Core biopsy Jan 08 showed; (posterior) ductal carcinoma in situ and calcification, (anterior) DCIS, lobular carcinoma in situ and calcification. S/P 3 lumpecties and S/P radiation treatment and now taking tamoxifen. July 08 F/U mammogram showed fine linear calcificatons in the area of prior biospsy. This area biopsied last week, results: Focal atypical ductal hyperplasis, fibrosis and chronic inlfammation with foreign body giant cells, consistent with previous biopsy site, clusters of ductal cells raise suspicion for invasive carcinoma, however immunohistochemistry for myoepthelial markers shows no evidence of malignancy.  MD recommending mastectomy with reconstructive surgery. Not sure what to do now? Is reconstructive surgery an option after radiation treatments?

Thanks for any information you can provide.

Hi there.

It seems that from your history, you really have a very high risk of developing invasive breast cancer in the future, and the recommendation of mastectomy with reconstruction is really plausible.

Reconstruction in a previously irradiated breast should offer no serious problems.  You and your doctors should just have to be more particular about wound healing and skin pliability and elasticity in the course of the reconstruction process (but as I said, with diligent care, there should be no problems).

Regards.

There is a new forum for reconstruction.  I posted a question there and received an immediate reply from the doctor.  You might want to repost there also.

This information has been helpful. I have decided and am scheduled for a mastectomy with reconstruction in a few weeks. The chest wall expander with implant will be the reconstruction procedure.  Thanks again for the helpful information.
