Really Serious?

This is an example of the letter I hand delivered to all the MDs in our area and I thought it might be something others might like to use.



I, __________________________________________________, (today’s date), request that I NOT be prescribed controlled substances unless absolutely necessary for my medical treatment and then only if my husband and I come in together for me to obtain such treatment.  Specifically but not limited to the following:

Narcotic Pain Relievers
Sleep Aids of any type
Tranquilizers and or sedatives
Narcotic Cough Suppressants

I have recognized that I have a problem with these medications and I am seeking to end the grip they have long had on my life.  Therefore I do not wish to receive any form of addicting medications. This has been a difficult path for me to take and I hope that as a medical provider you understand that this is a disease, but with the loving support of God, family, friends, and health care providers, I will be able to control this illness.

I was so lucky to have a supportive husband.  He knew how difficult it would be and he went with me to talk to each one of these doctors and support me.  It is just a little insurance for him and me that I can't call these docs or go to their office for a refill.  It really does help.
