This has been present for years already. These bumps on the scalp may be due to a fungal infection such as tinea capitis. It may also be due to psoriasis and seborrheic dermatitis. Discoid lupus may also present with scalp conditions. If hair loss is also noted, then these differentials that I have mentioned may need to be ruled out as they are associated with hair loss.
At this point, I would suggest a consult with a dermatologist. A skin biopsy may be able to help.
