Keep or kick Mirena?

I have had my mirena for nearly 2 years.  In that time I have had to go to the hospital due to extremely bad UTIs or kidney infections.  I never had a UTI in my life. I feel I've all but completely loss my libido and get yeast infections regularly.  I have had a serious loss of energy (but with 4 some kids and a full time job everyone thinks there's nothing odd about it- I disagree).
Because of cardiac history there are many BC options that I can't use.
My husband and I realized our previous form of BC wasn't working for us- we didn't plan any of the kids, so Mierna seemed perfect for us.  I am thinking it is not so right and don't know what to do.  I don't want sterilization but another kid may do us in as much as my lack of interest in sex is right now.
Help?  Suggestions?
