Brain MRI Results abnormal

by dad1313, Jul 25, 2008 04:25PM
Received my MRI of brain back  - doctor at walk in clinic who ordered referred me to neurologist - abnormal results...I'm concerned while awaiting to get into see neurologist - can you help me with interpretation of results?  Should I be concerned?

my symptoms -
I'm healthy 47 yr old male.  Last week I started having severe Migraine headaches 3 days in a row.  Since then I have a continued "pressure feeling" in my head but not the Migraines.  I do have continued sensitivity to light, sound, and touch.  Last week I also had and continue to have a "dull ache" in my upper chest area and shoulders and sometimes down into wrists and sometimes moves to mid to lower back.  I have a "hang over" feeling; fatigue; stiff neck and some dizziness.

I went to a walk in clinic last Thursday evening and they took bloodwork/labs including (lyme disease) all came back negative.  They also did chest xray, and ekg... both were normal.  They also sent me to get a MRI of Brain with and without contrast.  (those results are below)  They had me f/u with Neurology and Pulmonolgist.  Waiting to see Nerurologist to interpret results.  Pulmonologist said lungs are ok but did order CT of neck, spine and chest - had those tests done today...awaiting results.  (family history: 1 sister died at 36 yrs old after having headaches had anerysm (cause of death:    )  other sister has MS.  Both parents deceased.

Findings:
In the left frontal lobe, on axial images #12, there is a well-defined lesion measuring 1.2 cm in height x 1.2 cm in anterior to posterior demension x 1.2 cm in transverse dimension.  This is markedly hypointense on T2, FLAIR, and TI images.  There is a prominent vascular structure extending from the left frontal sub arachnoid space directly into this lesion, all of which demonstrate enhancement on postcontrast axial image 6-12.  There is no surrounding endema or mass effect.  I believe this likely represents a venous varix.
Adjacent to the right lateral ventricle, there is a 5 mm lesion that is mildly hyperintense on T2 and FLAIR images best seen on axial images #12 as well.  There are no other lesions.
No acute hemorrhages, infarcts, or extra-axial collections.  The ventricles are nondialated.  No mass effect or midline shift.
The brainstem and cerebellum are normal.  The IACs are normal.
There is prominence of the CSF spaces in the anterior temporal lobes bilaterally which likely represent benign arachnoid cysts, left greater than right.  On the left side, this measures 2.6 cm in anterior to posterior dimensions x 3.9 cm in transverse dimension and on the right side measures 1.2 cm in anterior to posterior dimension x 2.4 cm in transverse dimension.  no significant mass effect on the temporal horns.
The optic chiasm, sella, and orbits are normal.  the sinuses and mastoid air cells are clear.
Intracranial vessels maintain normal signal void.

Impression:
1.2 cm lesion in the left frontal lobe likely represents a venous varix.  There is no surrounding edema or mass effect.
5 mm lesion adjacent to the rt lateral ventricle can be seen in patients with migraine headaches, infectious/inflamm. etiologies, or auto immune diseases.  This lesion does not enhance.
Benign arachnoid cycts in the temporal lobes bilaterally, left greater than right.

If you could tell me your opinion of these results I would greatly appreciate it.
