very disruptive 9 year old

any suggestions on how to get a 4th grader to pay attention and not be completely disruptive in every class every day at school?  he is fine when he is with me but at school it is an every day thing to get into trouble.  not listening being disruptive, not doing work, talking back,
i have tried everything and nothing seems to work.  does anyone have a suggestion that might work?

My daughter and i are participating in a study at a local university for kids wi th ADHD.  It is a behaviior modification program, there are a few target behaviors that are worked on at a time.  The teacher sends home a Daily Report Card that list YES or NO for every part of the day reading, science, ss, special, lunch etc..... if the child gets a certain amount of yes's during that day they get to choose up to 3 rewards that evening.  Not something you BUY them, but something they enloy, such as computer time, helping with bakkng/cooking, special mom son time, video game time  or whatever works best.  Then on the weekend if they have x amount of good school days they get abigger reward such as a sleepover, go out to dinner, movie and popcorn night etc.... But they virtually start with nothing they have to earn everything.  My daughter is in third grade, this is working way better than i though that it would.  She really is a good kid and wants to get all of those yes's not just for the reward but because it truly makes her feel good.  The teacher readily complied with this.  And she sends me notes that this positive, structured environment really works for her.   It's worth a try.

Good feedback and also get her on some good muliple Vtamin -mineral supplemnets BComplex and Vit C and also Fish oil,
