Creatinin Level increased after 1st dialysis

My friend suudenly detected CKD having serum creatinin level 7.9 with 2 cyst in one kidney and 3 cyst in another kidney.Doctor advised immediate dialysis and advised for kidney transplantation.
After 1st dialysis the serum creatinin level found 16.His age 42 years,indian citizen.
Why it is happend and is it the case of CKD and is kidney transplantation require ?
