LASIK: Standard, Intralase & Epilasik - Share your experience

Hi everyone, I'm actually gathering some feedback about the different types of LASIK surgery that are available in the market today, and I'd like to hear from real life experiences from the public...people like you.
So feel free to reply to this thread, and share your lasik experience, and be sure to include the following:
1) Overall experience
2) What type of LASIK surgery - Standard, Intralase, Epilasik or others?
3) Total cost of surgery
4) Results
5) Feedback on surgeon, nurses, location, equipment, facility etc
6) Anything else you think readers should know
