High WBC R and L Side Pain, Nausea, Fatigue, Lightheadedness Please Help

I started having right and left side abdominal pain from the bottom of my ribs to my hip bone it spans a little in the front and sometimes I can feel it in my lower back some. (Not pelvic or stomach pain) It started on my right side first and a couple of weeks later it began on the left side too. It is at exactly the same spot on both sides. It feels like organ pain (Not pulled muscles or bone pain) . It feels as though someone were standing behind me clenching each side as hard as they could. It all started around July 10th .Went to ER, urine test was normal. Cat scan showed nothing. All was normal but a small ovarian cyst and a high wbc count. Went to doctor ran test again was even higher (went from 13.5 to 16). Other blood tests were normal again. Ive tried 2 antibiotics with no improvement. Pain is dull and comes and goes. Although the last week or two its been pretty much constant only relenting for maybe an hour or two. My resistance to the pain is weakening. It hurts. I am severely and constantly fatigued . I also have constant nausea most of the time. I never actually vomit. Sometimes I have heartburn. I havent had any fever. When I try to walk through a grocery store for too long or bend over and stand up from weeding garden I get really woozy and lightheaded. It feels like Im going to pass out but if i sit really quick and wait a few minutes later it will pass. Nothings spinning but I get really weak. The doctor is sending me to get a cbc again and see if white blood cell has gone down any, if not then hell send me to a hematologist. He said that theyd run tests on me including a bone marrow test. Its beginning to scare me. Im starting to get depressed because I dont have the energy that I used to.. I was a very active person and I have people depending on me to take care of them and its getting to the point that i can barely take care of myself.
Im in my mid twenties, overweight, and I smoke but generally healthy.

Im desperate for any suggestions or opinions
Please Help if you can.

Thanks for the detailed post and I am sorry I was late in replying, adding to your anxiety.
Abdominal pain that radiates to the back can be due to gall bladder problems like cholecystitis and biliary colic. However this pain is only on right side of abdomen. Pain on both sides could be due to involvement of kidney. Similarly pain due to abdominal aortic aneurysm, pancreatitis, aortic dissection and duodenal ulcer may appear as both side pain or pain starting in one side and moving to the other and to the back. In your case it could be a combination of gallbladder problem and duodenal ulcer problem. I understand a CT has been done. Was it whole abdomen?

High WBC count is generally seen in infection, but could also be due to a tumor. It definitely needs further investigations. In your case even the small ovarian cyst should be further investigated as ovarion tumors and cysts that get tortutous too mimick an abdominal pain that may radiate to the back.

You may have to undergo some tests like kidney and liver function tests, some hormone analysis as you have an ovarian cyst, abdominal ultrasound, differential count of WBC to know the types of WBC and a peripheral smear to see the conditions of the cells in the blood to reach a diagnosis. A clinical examination by a doctor will also be very helpful.

However it is difficult to diagnose without examining. So please consult your doctor. Hope this helps. Please let me know if there is any thing else.
