Abdominal pain that radiates to the back can be due to gall bladder problems like cholecystitis and biliary colic. However this pain is only on right side of abdomen. Pain on both sides could be due to involvement of kidney. Similarly pain due to abdominal aortic aneurysm, pancreatitis, aortic dissection and duodenal ulcer may appear as both side pain or pain starting in one side and moving to the other and to the back. In your case it could be a combination of gallbladder problem and duodenal ulcer problem. I understand a CT has been done. Was it whole abdomen?
High WBC count is generally seen in infection, but could also be due to a tumor. It definitely needs further investigations. In your case even the small ovarian cyst should be further investigated as ovarion tumors and cysts that get tortutous too mimick an abdominal pain that may radiate to the back.
You may have to undergo some tests like kidney and liver function tests, some hormone analysis as you have an ovarian cyst, abdominal ultrasound, differential count of WBC to know the types of WBC and a peripheral smear to see the conditions of the cells in the blood to reach a diagnosis. A clinical examination by a doctor will also be very helpful.
