drastic changes in penis

Hello;I'm 54 year old male.
I have gone to a dermatologist and an urologist - neither if the can pin point my condition; about a year ago I noticed a discoloration around the top of the penis .Also noticed some round bumps ,a bit reddish around the circumcised part of my penis. This areas would itch off and on - I thought it would be due to dryness ,so I used body lotion.it would feelbetter ,but the disconfort would prevail.I have been with the same person for the past 20 years ,so other types of stds are ruled out. Now the areas where the skin discoloration is ocurring feels like it is thinning and it hurts during intercourse . Feels like it is about to tear , a very painful and anoying sensation.
the dermatologist ruled out vitiligio , although acknowledged the continous itch.The only medication I have used during this time has been adipex 37.5 and Zoloft 50 mg. both of them off and on during the past year and a half.
Could ayone give me some insight please?
