October 2005
• Severe back pain all over my back nothing would relieve it. Lasted 1 week.
• Hip pain worst on the left side.
January 2006
• Severe back pain returns.
• Back spasms.
March 2006
• Back pain returns. It is even worse. It would spasm and burn. It hurt so bad I could not even sit or stand up. I had to have assistance from my husband a number of times just so I could get home. This lasted for a little over 2 months almost 3.
• Tingling from the waist down.
• Trembling of hands when doing sensitive work
• Started Flexoril for back spasms.
• Tests were done for Lupus, Mono, Shingles, Hepatitis C, and there may have been more. All were negative.
April 2006
• The back pain is continued from March 2006.
• Tingling in the legs has spread to arms right side then left side.
• I felt a tightness around my torso making it difficult to breath. This came and went for a couple of months.
• I felt disoriented and my brain would feel fuzzy (confused). This lasted a few days.
• Went to ER found out allergic to Flexoril
• Tried Soma found out allergic
• Had an MRI of Brain, C, T, and L spine. First Neuro said it was clean. Reports from radiologist mostly stated clean with one bulging disc. CT scans were also done.
May 2006
• Back Pain still continued from March
• Tiredness. I constantly wanted to sleep and did not have much energy to do things.
• Started Gabapentin given by first neuro. This seemed to relieve a lot of the back pain and tingling.
• Symptoms gone by end of may mid June
July 2006
• Tightness in chest on and off. Made breathing difficult
• Became pregnant.
• Fainted or passed out a few times
• Symptoms left by Mid Aug
March 2007

• Back pain and spasms. Burning sensation in my back.
• Tingling from waist down
• Trembling of hands when doing sensitive work

March 2008
• Back pain, spasms and burning sensation returns. Back became tender to the touch,
• Tingling from the waist down.
• Hip pain (left side) acted up.
April 2008
• Symptoms from March continue.
• UTI
• Treated with prednisone. Took for 1 week. Didn’t help
May 2008
• Symptoms from March continue.
• Tingling in legs spread to back and arms.
• Leg pain (worse on right side). Sensitivity to touch in the legs, especially right hamstring. Legs constantly felt stiff and sore like I had run marathon. I thought I would collapse between how weak they felt and the pain. Lasted 1 week.
• Pain in my arms (worse on the right side) Arms felt heavy and tired (worse on the right) Pain in my right elbow. Pain in hands so severe that I could not type or play piano. Still occurring off and on, but originally lasted for a week straight.
• Major fatigue. Much worse that last time. Rest would not reduce it. Relaxing did not help. Wanted so much to fall asleep but could only stay asleep for a couple hours at a time.
• Inability to multitask like a used too. Can’t find the word I need. Harder time focusing on a task.
• Xrays of my back were done and nothing was found.
• Given Arthrotec and Lyrica. Took for 3 days before my GP took me off of it. Went back onto my Gabapentin.
• Symptoms lessened at the end of May. Most have left completely some are still there but less intense. (This Note Written June 14 2008)

Mid-July 2008
• Tingling all over and now in my neck.
• Vision problems ( it seems hard to focus my vision, like blurry or moving vision)
• Burning back legs hands. The burning is getting worse in the feet (new)
• Joints hurt all over (hands, elbows, knees)
• Weakness
• Fatigue
• Loss of coordination. It feels like the floor is moving around when I walk sometimes. Hand eye coordination makes it harder to do things. I went to sit down on toilet and almost completely missed it.
Before 2005 I was very active. I ran Cross Country and Track in high school. My freshman year of college I ran a half marathon with my mother. I was running that fall.
