I am so sorry for what you are going through but it does sound like you should go forward with this second surgery. Not sure what happened during your first surgery but given your pain, the most recent ultrasound showing the cyst is there, I would recommend surgery as well. Hope that helps.

It is likely that this pain is from where you are healing from your original surgery (laparotomy) but I would recommend you make an appointment with your gynecologist for exam/evaluation.

I would speak to your OB about this cyst. The cyst in itself does no cause miscarriage. There is a small chance of the cyst rupturing or your ovary twisting in which case you would need surgery.

I would recommend repeating PUS or obtaining pelvic MRI to get further information. As above could also just go straight to surgery as then you will know 100%.

Not sure what all of this is caused by. I would recommend calling your primary doctor and being seen again.

Given your family history, the US report, and your symptoms I think the plan for US followup in June is good. I usually re-image 6-8 weeks after an US of this sort. Given this US report it seems you tend to make cysts in your ovaries, you have a hemorrhagic cyst which may have ruptured as there was fluid in your pelvis, and that it should be followed by rep...
