There are several causes of enlarged and painful testicle including infection, inflammation, and tumor. Since you have a history of testicular mass in the past, you should be checked out for all the above. Good luck.

Several things can cause blood in semen (called hematospermia). Most of the time it is self-limited and goes away on it's own without any specific cause or treatment. Usually inflammation of the prostate/seminal vesicle or post-biopsy can be causes.

Glad to help interpret the CT findings. Actually Hypertrophy is enlarged and atrophy is shrunken, therefore the left kidey is the large one and likely compensating for the right kidney.
