.Hello. I am a 35 year old female with a 10 year history of PSVT controlled to a degree with Atenolol 25 mg 2x/day. I also have PVC's and PAC's. More recently, I went to my cardiologist because I was having 3 PVC's in a row then a sudden run of tachycardia. It made me dizzy and shaky to the point I almost blacked out. The ECG revealed a "new IRBBB (incomplete right bundle branch block) and "probable atrial enlargement". He simply says, "Probable left atrial abnormality with IRBBB, which is new. Patient was concerned and we reassured her." I cannot tolerate exercise that is strenous as my heart rate increases with a walk (even on the medicine).I am scared. I called him after I received my records and he reassured me again that IRBBB is a normal variant and probably came from my years of SVT.??? He said not to worry. He has NEVER done a stress test or cardiolite or anything except the initial echo in 1998. Several holters and event monitors, however, have revealed nothing more than SVT. I am wondering if the "new" findings could in fact represent something more going on. I've read that left atrial enlargement with IRBBB can mask LVH and that usually when found (atrial enlg)predicts LVH as well. I am fatigued ALL the time. Please let me know what you think about the "new" finding and if I should seek a second opinion. I have a copy of my ECG scanned if you want to look at it.
