2004 - chronic active hepatitis with mild to moderate activity and focal early periportal fibrosis - Grade 1/Stage 2; lab note: reviewed the slides from 1999; in comparing the biopsies, note that the changes are slightly more advance in the current biopsy that the changes present in the biopsy from 1999. This would apply to both the degree of activty and the fibrosis present currently.

Prior treatment 2006 - interferon and ribaviron, VL start 3 million, dropped to 4000 week 12; up to 14,000 week 24 and stopped treatment
Current treatment; triple tx with incivek, 1/13/2012. VL start 3 million and UND at week 4
3/7/12 Sx: anemia and dropped riba to 600 mg, holding steady today at 11.6; gnarly rash began at week 7 and going strong
