you can also get this situation with a few other rare medical conditions.
in italy where I live they alays test all 3 things but in the UK they save money by only testing TSH, so you'd have to get a private test to check it.

T3 is the hormone actually produced by the thyroid gland. various organs around thebody convert that into T4, as needed.
TSH is made in the brain and, in most cases, goes up if your thyroid gland is not producing enough T3 - your brain tries to make the thyroid work harder to produce more of the hormone.
This is why most people with hypothyroid have abnormally high TSH.
Some people, mainly ones with Lyme disease, usually get low thyroid because there are lyme bacteria in their thyroid gland. Unfortunately they get them in their brain too, and so the TSH level does NOT go up in response to the low level of T3. that means, if you test only TSH, everyhting looks normal!
