Methadone is a very long acting opiate and I would suggest that he is off of it for at least 2(two) weeks prior to the first dose of Naltrexone. Otherwise he may experience severe withdrawal symptoms. Is Vivitrol injection (one month depot shot) available in South Africa? That is the best way of therapy if an implant is unavailable. You are absolutely right that substituting an illegal opiate with methadone does not make one clean. Methadone is still a heavy duty drug and extremely hard to get off of.
Now. you asked about Ibogaine. There are some clinics in South Africa as well as other countries experimenting with this drug. You may be able to find one near you.
Since I am in the US, and it is illegal here, I will not pretend to know very much about it. I do know that whereas it has been shown to be useful in detox, it also has been found to have many dangerous side effects including cardiac arrhythmias and neuronal degeneration.
