Itchy Hair Loss all over the head, causes ?

Hey there people.Before i start id like to mention that ONLY grandpa from mother's side has some frontal hair loss but its almost unnoticable,and both grandma's had some hair loss.Even grandfathers died with thick hair which means i had no history of MPB.My hair was thinning for the past 2 years,when the hair is about to fall out i get strong tingly/itchy sensation on that spot and if i pull my hairs i can see bunch of hairs,sometimes its one and sometimes there are 6-7 hairs and all of them have white bulb at the end(They never seem to grow back).Those hairs look healthy and thick,thinner hairs are rare.and i have no receding hair line its just thinning mostly on the top,sometimes i get the itchy scalp on sides and back and im able to pull out hairs easily without pain.I have also noticed that my eyelashes/eyebrows and chest hair can be easily pulled out.What pissed me off is that im only 21 and my hair is thinning,is this MPB or something else ? Thanks a lot
