Do counselors "diagnose" a condition when billing insurance?

I have been seeing a licensed counselor about some minor depression I am having over some recent events in my life. I refuse to take medication (and really don't need it) because it will be on my record of having a mental health issue and I don't want that for jobs I could potentially apply for.. ie police officer, military etc..

I have been seeing a licences counselor, not psychologist or anything about my issues. When they bill our sessions to my insurance, do they have to list an issue im seeing them for? Like if they list depression, will it go on my medical record that i have depression?

Yes.  Insurance companies determine how much they pay and what they pay by codes that include diagnosis.  This information is required by law to be kept private, so unless you tell someone they shouldn't know.
